The 1st row,"BROWSER Forms", open and load quickly. The 2nd row open in "Microsoft Word" and can be altered, but they open slow. The 3rd row also opens in "Microsoft Word", but these Forms have been computer traslated into Spanish.
Remember: This page only contains the forms for collecting the patient's answers! You need to return to the Software Section to compile the narrative, self-scoring reports.
