CMS 1500 (Formerly called HCFA 1500), 2 Part Continuous Feed, with NPI number. 1000 forms per case. This is a 2-Part continuous Feed form used with a special printer that has tracks for the feeds on the side of the form. The 2nd part is carbon copy, which is often placed in the patient's file for documentation. You CAN NOT use a standard printer with this form!
